Most of the time if you are maintaining your Child’s yearly check up, a separate sport physical may not be required since your provider had done a Physical exam within the same year. In some scenario or if you are a new patient to the practice you may need to have a new Sports Physical to be able to participate in the sports. In order to qualify your participation do not forget to provide us with your child’s Sports Physical clearance form that is required to be completed by your child’s primary care provider.
